The New Medical Examiner Service

Overview of the Medical Examiner Service

  • Established following key recommendations to improve the review process for deaths.
  • Ensures transparency, accurate certification, referrals to coroners where necessary, and improved healthcare practices.

Key Roles

  1. Medical Examiners: Senior doctors trained in legal and clinical death certification processes.
    • Advise on coroner referrals.
    • Assist doctors in certifying causes of death.
    • Identify healthcare improvements.
  2. Medical Examiner Officers: Specialists in healthcare and certification systems who manage cases and act as the first point of contact for next of kin.

Process Steps Post Death

  1. GP notifies the Medical Examiner’s Office.
  2. Medical Examiner reviews the death and contacts the next of kin to explain the cause of death and address concerns.
  3. Information is sent to the GP to complete the medical certificate of cause of death (MCCD).
  4. MCCD is approved by the Medical Examiner and sent to the Registry Office.
  5. Next of kin is informed and can schedule death registration.

Interaction with Next of Kin

  • Next of kin can discuss the cause of death and raise concerns.
  • Questions about medical care and terminology are addressed to improve understanding and future care.
  • Speaking to the Medical Examiner Office is encouraged but optional.

Coroner’s Involvement

  • Some deaths must be legally referred to a coroner, with the Medical Examiner providing medical advice as needed.

Concerns and Feedback

  • Feedback on the service is welcomed via email.
  • Unresolved issues can be escalated to the Patient and Liaison Service (PALS).
